CPC's Case Management program (ICMS) assists individuals with serious and persistent mental illness in the adaptation to community living. Since 1997, CPC has helped adult consumers manage their illness and secure the necessary services and resources to improve their quality of life.


The goal of the program is to prevent unnecessary hospitalizations and help the consumer live a productive life in the least restrictive setting possible. Case managers work collaboratively with the consumer and his/her support network, including all providers of treatment, to ensure that necessary and appropriate services are received.

Consumers who are eligible include:
  • Those recently released from a state psychiatric hospital or short-term psychiatric units in local medical hospitals
  • Those who frequently use hospital emergency rooms for psychiatric crises
  • Those with serious and persistent mental illness who have not benefited from traditional outpatient services


Services include:
  • Consumer-centered and individualized services in the consumer's home environment
  • Assessment/Service Planning: varying degrees of service intensity based on the consumer's changing needs, for up to 18 months
  • Linkage to Services: linking consumers to community services such as psychiatric and medical care
  • Assistance with obtaining basic living necessities
  • Coordination of Care: partnering with the consumer's family and/or significant others to provide care
  • Assertive, intensive care for consumers who have not succeeded in traditional treatment environments
  • Crisis Intervention
  • Advocacy
  • Medication Education
  • Supportive Counseling
  • Family Education and Support

The ICMS office is located at CPC's Helen Herrmann Counseling Center in Middletown/Red Bank.
